Your Personal Data
On the next page you are asked to provide a few details about yourself. You are then asked to complete a health questionnaire. Your questionnaire will not automatically be seen by your Doctor or GP practice. Therefore, if any of the questions or the information contained in this Staywell programme give you any cause for concern, please make an appointment to discuss your worries with your Practice Nurse or GP.
When you provide personal information it will be kept secure and will only be accessible to the members of the health care team providing your care.

Please remember, you are under no obligation to use the Staywell service. However, it is hoped you will do so because it may help you understand more about your condition and how to manage it.
Completing the Questionnaire
Although there may seem to be a lot of questions to answer, please don’t be put off. The questions are very simple and take most people around 10-15 minutes to complete.
You can answer yes, no or not sure to each question. Please do not worry if “not sure” is the answer you feel you have to give. It’s not an exam!
