When completing the Staywell programme you can make a choice. You can either give us your name and some contact details, in case we might be able to offer you some self management support one day, or you can choose to remain anonymous.
If you are happy to give us your contact details, one of the things we need to know is which doctor’s practice you belong to.
Please locate your practice on the list below and click on the name to highlight it.
Then click the “Continue” button.
Alternatively, if you wish to proceed anonymously that’s fine, simply select the “Anonymous” button now.
But please remember, if we don’t know who you are we can’t offer you any support.
