Staywell

Intouch has developed a new service called "Staywell" focused on self care education for patients with long-term conditions. As well as empowering patients and improving their confidence and quality of life, Staywell self-care tools is designed to reduce their complications and exacerbations. It is therefore a key element in unscheduled care strategies, the avoidance of hospital admissions and the shift of care out of the acute setting and into the community.

The Staywell self-care education and tools range currently covers Generic Conditions, COPD , Diabetes and Hypertension

Also Staywell Medication provides a review tool asking patients about their medication and how they use it or not as the case may be!

A special version of the service, Staywell 75, is designed to identify early stage, emerging health risks in older patients which are likely to result in hospitalisation if not managed in good time, as is Staywell 65
